Cramp Bark (Viburnum opulus) is one of the most specific smooth muscle antispasmodics in the Western herbal pharmacopoeia — its traditional application centered on the uterus, but its mechanism is broader. The primary bioactive compounds include scopoletin (a coumarin derivative) and viopudial (a bicyclic iridoid), both of which act as calcium channel antagonists in smooth muscle cells. Smooth muscle contraction requires an influx of calcium through voltage-gated calcium channels — by modulating calcium entry, cramp bark reduces the frequency and intensity of involuntary contractions. This is different from over-the-counter analgesics, which address pain perception; cramp bark addresses the underlying contractile activity. Isovaleric acid (a volatile fatty acid) also contributes mild CNS-mediated relaxation. The antispasmodic action extends to smooth muscle throughout the body — cramp bark is used in Western herbalism for intestinal cramping, urinary urgency, and musculoskeletal tension alongside its primary uterine application.
