Prepared rehmannia (Rehmannia glutinosa radix praeparata — Shu Di Huang) is one of the most foundational herbs in the Chinese materia medica. The distinction between "prepared" (Shu Di Huang) and "raw" (Sheng Di Huang) rehmannia is pharmacologically significant: raw rehmannia is cooling, used for heat patterns and acute inflammatory conditions; prepared rehmannia (steamed nine times with rice wine in the classical method) is warming and tonifying, concentrated in the oligosaccharides, catalpol content, and acteoside that make it appropriate for long-term nourishing support of kidney yin and liver blood. Catalpol is an iridoid glycoside — research has shown it upregulates BDNF (brain-derived neurotrophic factor) in the hippocampus, providing neuroprotective effects studied in models of cognitive aging and neurodegeneration. Acteoside (verbascoside), a phenylethanoid glycoside, provides potent antioxidant and anti-inflammatory activity. Together these constituents underlie rehmannia's traditional role as a tonic for deep deficiency: low back and knee weakness (kidney yang/yin deficiency pattern), night sweats, tinnitus, and premature aging.
